complete them and return them to HSA. You'll be contacted by one of our Regional
Coordinators to discuss some set-up details and we'll get you ready to receive referrals!
The forms include:
- A Service Contractor Profile / General Information form.
- A form to capture the types of services on which you'd like to receive referrals.
- Instructions on what to send HSA with regard to licensing, insurance and 1099 reporting.
